Hi Erik,

I've been reworking the filter a bit, favoring more recent amavis 
releases.  I believe you are running and older version (via maia 
mailguard).  Would it be possible for you to send a log to me?

I've resolved the other two problems.  I'll post when I get complete the 
fix for your entry 3 (your log will help ensure I catch all the older 
entries).
